By attending, participating in, or engaging in any program, event, class, workshop, or activity hosted by JACCK Foundation for the Arts, I grant permission to JACCK Foundation for the Arts, its representatives, employees, volunteers, contractors, and assigns to photograph, record, and capture my image, likeness, voice, and/or appearance in photographs, video recordings, audio recordings, and other media formats.

I understand and agree that such media may be used, reproduced, edited, published, displayed, distributed, and shared—without limitation—for promotional, educational, marketing, fundraising, archival, and informational purposes in any medium now known or later developed, including but not limited to websites, social media, print materials, advertisements,…
